Please Note: A CDL (Commercial Drivers License) Holder may not elect school on any citation.

Please Note: Any driver charged with exceeding the speed limit by 30 M.P.H. or over MAY NOT elect traffic school and there is a MANDATORY court appearance.

I hereby swear or affirm that, subject to the penalty of Perjury and possible Contempt of Court, as of the date, I have not elected as set out in 318.14(9)(a), within the preceding 12 months or more than 5 times in a 10 year period, to have points withheld from my driving record by attending a Driver Improvement School approved by the State in lieu of payment of any civil penalty or court appearance.

FURTHER, I elect to attend a Driver Improvement School approved by the State of Florida, and shall provide a certificate of completion of Driver Improvement School to the County Clerk's Traffic Division (in the county in which I received the citation), within 90 days after receipt of this affidavit. Failure to comply will result in forfeiture of your election for school and points will be assessed against your driving record.

I understand that, if I do not complete my Driver Improvement School as scheduled, points will be assessed against me, my driving privileges will be suspended and I will be required to pay a processing fee plus the late fee.
